    目的:探讨变应性鼻炎患者鼻塞主客观评估与心理状态的相关性,分析主客观不一致患者的特征。方法:本研究回顾了北京市海淀医院自2025年1月至2025年10月期间的变应性鼻炎患者100例,采用视觉模拟评分(visual analogue scale,VAS)、鼻结膜炎生命质量调查问卷(rhinoconjunctivitis quality of life questionnaire,RQLQ)、鼻症状总积分(total nasal symptom score,TNSS)、鼻炎伴随症状总分(total non-nasal symptom score,TNNSS)评估主观症状;采用抑郁自评量表(self-rating depression scale,SDS)、焦虑自评量表(self-rating anxiety scale,SAS)评估心理状态;通过鼻腔有效阻力、鼻腔容积、鼻腔最小横截面积等指标评估客观鼻通气功能。采用Spearman相关性分析探讨各指标间的相关性。结果:患者VAS评分为6.18±2.35分,RQLQ总分为86.20±36.46分,SDS标准分为50.28±11.74分,SAS标准分为45.99±11.42分。鼻腔有效阻力为1.00±0.38 Pa/(cm3·s),鼻腔容积0~7 cm为8.75±3.52 cm3,鼻腔容积2~6 cm为4.93±2.29 cm3,鼻腔最小横截面积为0.43±0.27 cm2。Spearman相关性分析显示,SDS标准分与SAS标准分呈高度正相关(r=0.807, P<0.01),RQLQ总分与TNSS评分(r=0.646, P<0.01)、VAS评分(r=0.585, P<0.01)、SAS标准分(r=0.578, P<0.01)及SDS标准分(r=0.538, P<0.01)均呈显著正相关。在主客观鼻塞不一致患者中,RQLQ总分与SDS标准分呈正相关(r=0.476, P<0.05),与SAS标准分呈正相关(r=0.524, P<0.01),鼻腔容积与心理状态评分呈显著负相关(r=-0.650至-0.662, P<0.01)。结论:变应性鼻炎患者的生活质量与心理状态密切相关,主客观鼻塞不一致患者表现出独特的相关性模式,客观鼻腔容积与心理状态呈显著负相关,提示心理因素在鼻塞感知中发挥重要作用。

    Abstract:

    Objective: To investigate the correlation between subjective and objective assessments of nasal obstruction and psychological status in patients with allergic rhinitis, and to analyze the characteristics of patients with subjective-objective discordance. Methods: This retrospective study included 100 patients with allergic rhinitis from Beijing Haidian Hospital between January 2025 and October 2025. Subjective symptoms were assessed using the visual analogue scale (VAS), rhinoconjunctivitis quality of life questionnaire (RQLQ), total nasal symptom score (TNSS), and total non-nasal symptom score (TNNSS). Psychological status was evaluated using the self-rating depression scale (SDS) and self-rating anxiety scale (SAS). Objective nasal ventilation function was assessed through nasal effective resistance, nasal cavity volume, and minimal cross-sectional area. Spearman correlation analysis was employed to explore the relationships among these indicators. Results: The VAS score was 6.18±2.35, RQLQ total score was 86.20±36.46, SDS standard score was 50.28±11.74, and SAS standard score was 45.99±11.42. The nasal effective resistance was 1.00±0.38 Pa/(cm3·s), nasal cavity volume 0-7 cm was 8.75±3.52 cm3, nasal cavity volume 2-6 cm was 4.93±2.29 cm3, and minimal cross-sectional area was 0.43±0.27 cm2. Spearman correlation analysis revealed a strong positive correlation between SDS and SAS standard scores (r=0.807, P<0.01). RQLQ total score showed significant positive correlations with TNSS score (r=0.646, P<0.01), VAS score (r=0.585, P<0.01), SAS standard score (r=0.578, P<0.01), and SDS standard score (r=0.538, P<0.01). In patients with subjective-objective discordance, RQLQ total score was positively correlated with SDS standard score (r=0.476, P<0.05) and SAS standard score (r=0.524, P<0.01), while nasal cavity volume showed significant negative correlations with psychological status scores (r=-0.650 to -0.662, P<0.01). Conclusion: Quality of life is closely associated with psychological status in patients with allergic rhinitis. Patients with subjective-objective discordance exhibit a distinctive correlation pattern, with objective nasal cavity volume showing significant negative correlations with psychological status, suggesting that psychological factors play an important role in nasal obstruction perception.
