    目的 探讨天门市鼻出血患者的就诊特征,分析空气污染水平及气象因素对住院手术风险的影响,为临床预防与公共卫生干预提供依据。方法 回顾性分析2022年1月1日—2024年12月31日在天门市第一人民医院就诊的鼻出血患者资料,包括性别、年龄、就诊时间及住院手术情况。同期空气污染物数据来源于湖北省生态环境厅,气象数据来源于美国国家海洋和大气管理局。对环境因素进行描述性统计与Spearman相关分析,以住院手术为结局变量,开展单因素与多因素 Logistic 回归分析。结果 共纳入鼻出血患者9 803例,同期PM2.5、PM10、O3平均浓度分别为36.07 、71.17 、103.18 μg/m3,SO2、CO、NO2平均值分别为7.06 μg/m3、0.73 mg/m3、16.41 μg/m3,空气质量指数(AQI)平均值为75.85,平均气温18.48℃,平均风速4.19 m/s,平均湿度中位数 56.80%。Spearman分析显示PM2.5与PM10相关性最强(r=0.91);多数空气污染物浓度与平均气温、平均湿度及平均风速呈负相关(r=-0.18~-0.64);就诊人数与平均气温及平均湿度呈正相关(r=0.32~0.40),与空气污染物呈负相关(r=-0.26~-0.04)。就诊人数呈春夏高、秋冬低趋势。AQI分级显示,多数患者就诊日的空气质量分布于空气质量“优”和“良”(7 892例)。多因素回归分析显示,PM2.5、平均气温和平均风速可降低住院手术风险(OR=0.988、0.952、0.954);SO2(OR=1.089)和CO(OR=1.762)增加住院手术风险;中度污染(OR=3.064)和重度污染(OR=4.348)显著提高手术概率。结论 天门市鼻出血具有显著的时间分布和人群特征,空气污染与气象条件对住院手术风险具有重要影响,控制SO2、CO水平并改善整体空气质量有望降低鼻出血严重程度及手术风险。

    Abstract:

    Objective To explore the clinical characteristics of patients with epistaxis in Tianmen City, and analyze the influence of air pollution levels and meteorological factors on the risk of inpatient surgery, so as to provide a basis for clinical prevention and public health intervention. Methods A retrospective study was conducted to collect the data of patients with epistaxis who visited Tianmen First People’s Hospital from January 1, 2022 to December 31, 2024, including gender, age, time of visit and hospitalization surgery situation. The air pollutant and meteorological data during the same period were obtained from the Hubei Provincial Department of Ecology and Environment, the National Oceanic and Atmospheric Administration of the United States. Descriptive statistics and Spearman correlation analyses were performed on environmental factors. Univariate and multivariate logistic regression analyses were conducted using inpatient surgery as the outcome variable. Results A total of 9 803 cases with epistaxis were included. During the same period, the average concentrations of PM2.5, PM10, and O3 were 36.07, 71.17, and 103.18 μg/m3, respectively, while the average values of SO2, CO, and NO2 were 7.06 μg/m3, 0.73 mg/m3, and 16.41 μg/m3 respectively. The average air quality index (AQI) was 75.85, the average temperature was 18.48℃, the average wind speed 4.19 m/s, and median humidity was 56.8%. The Spearman correlation analysis showed that PM2.5 had the strongest correlation with PM10 (r=0.91), and the concentrations of most air pollutants were negatively correlated with average temperature, average humidity, and average wind speed (r=-0.18 to -0.64); the number of patients seeking medical treatment was positively correlated with average temperature and average humidity (r=0.32 to -0.40), and negatively correlated with air pollutants (r=-0.26 to -0.04). The number of patients seeking medical treatment showed a trend of being higher in spring and summer and lower in autumn and winter. The AQI classification indicated that the air quality on the days when most patients (7 892 cases) sought medical treatment was distributed in the “good” and “excellent” categories. Multivariate regression analysis showed that PM2.5, average temperature, and average wind speed reduced the risk of hospitalization for surgery (OR=0.988, 0.952, 0.954), while SO2 (OR=1.089) and CO (OR=1.762) increased the risk. Moderate (OR=3.064) and severe pollution (OR=4.348) significantly increased the probability of surgery. Conclusions Epistaxis in Tianmen City shows distinct temporal distribution and demographic characteristics, with air pollution and meteorological factors significantly affecting the risk of hospitalization and surgery. Controlling the levels of SO2 and CO and improving overall air quality are expected to reduce the severity of epistaxis and the risk of surgery.
